Patient-centered care emphasizes respect for patient values in individual care decisions as well as the role of patients and families as advisors and essential partners in improving care practices. We identify that each patient and each family is unique. Families have distinctive personalities, values, beliefs, and religious and cultural backgrounds. Care provided should be equal for all patients and flexible so that the needs and choices of families can be met.
Case Management
Upon discharge, ATLAC Case Managers work in partnership with insurance case managers, social workers, doctors, therapists, and hospital discharge planners to make certain that utmost benefits are obtained for each client. Closely working with everyone in the care management of our client is top priority. Once our patients are admitted into the comfort of their home, we continue to work on ensuring that care and benefits are maintained. In order to maintain continuity of care with skilled nursing, ATLAC staff works closely with DME (Durable Medical Equipment) companies to organize for any necessary medical equipment and supplies needed.

Help from our registered nurses can help your loved one go home sooner after a hospital stay or surgery.
